บทคัดย่อ

บทนำ: โรคหลอดเลือดสมองเป็นสาเหตุของความพิการและภาวะพึ่งพิงทั่วโลก การดูแลต่อเนื่องที่บ้านมีความสำคัญอย่างยิ่งในการป้องกันภาวะแทรกซ้อน ส่งเสริมการฟื้นตัว และเพิ่มคุณภาพชีวิตของผู้ป่วย

วัตถุประสงค์การวิจัย: เพื่อศึกษาสถานการณ์ พัฒนาและประเมินรูปแบบการดูแลต่อเนื่องทางการพยาบาลผู้ป่วยโรคหลอดเลือดสมองที่บ้าน โดยการมีส่วนร่วมของครอบครัว

ระเบียบวิธีวิจัย: การวิจัยเชิงปฏิบัติการนี้ แบ่งเป็น 3 ระยะ ได้แก่ 1) การศึกษาสถานการณ์และปัญหา 2) การพัฒนารูปแบบ และ 3) การติดตามและประเมินผล โดยใช้กรอบแนวคิดแบบแผนความเชื่อด้านสุขภาพ และการมีส่วนร่วมของครอบครัว กลุ่มตัวอย่าง ประกอบด้วย ผู้ป่วย 60 ราย ผู้ดูแลหลัก 60 ราย และบุคลากรสหวิชาชีพ 25 ราย คัดเลือกแบบเจาะจงจากโรงพยาบาลแม่ข่าย 5 แห่ง เครื่องมือวิจัย ได้แก่แบบสอบถามความรู้ ความเชื่อ ทัศนคติ พฤติกรรมการดูแล แบบประเมินกิจวัตรประจำวัน การสัมภาษณ์เชิงลึก และแบบประเมินความพึงพอใจ ตรวจสอบความตรงเชิงเนื้อหา ได้ค่า IOC 0.67 – 1.00 ค่าความเชื่อมั่นของแบบสอบถาม .79 และแบบทดสอบความรู้ .80 วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา และ Paired t-test ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เนื้อหา

ผลการวิจัย: รูปแบบการดูแลด้วยกิจกรรมส่งเสริมการรับรู้ความสามารถของตนเอง และกิจกรรมเล่าสู่กันฟังผ่านสมุดบันทึกการดูแลต่อเนื่องทางการพยาบาล ส่งผลให้ผู้ดูแลหลักมีความรู้ ความเข้าใจ การรับรู้ด้านสุขภาพและพฤติกรรมการดูแลผู้ป่วยดีขึ้น อย่างมีนัยสำคัญทางสถิติ (p < .001)

สรุปผล: รูปแบบดังกล่าวช่วยเสริมสร้างศักยภาพผู้ดูแลตามแนวคิดแบบแผนความเชื่อด้านสุขภาพ และการมีส่วนร่วมของครอบครัว ทั้งด้านการรับรู้ประโยชน์ ความสามารถ และลดอุปสรรคในการดูแล ส่งผลให้ผู้ป่วยมีแนวโน้มพึ่งพาตนเองในการทำกิจวัตรประจำวันได้มากขึ้น

ข้อเสนอแนะ: ควรขยายผลการใช้รูปแบบไปยังหน่วยบริการสุขภาพอื่น โดยปรับตามบริบทพื้นที่ และสนับสนุนการอบรมเชิงปฏิบัติการแก่ผู้ดูแลในชุมชนอย่างต่อเนื่อง
