บทคัดย่อ

บทนำ: ภาวะติดเชื้อในกระแสเลือดของทารกแรกเกิด เป็นสาเหตุสำคัญของการตายและความพิการในทารกแรกเกิด

วัตถุประสงค์การวิจัย: เพื่อพัฒนาแนวทางการดูแลรักษาภาวะติดเชื้อระยะแรกของทารกแรกเกิด และประเมินประสิทธิผลของการพัฒนาแนวทางการดูแลรักษาฯ โรงพยาบาลโพนพิสัย จังหวัดหนองคาย

ระเบียบการวิจัย: การวิจัยเชิงปฏิบัติการโดยใช้แนวคิดของเคมมิสและแมกทากาด กลุ่มตัวอย่าง จำนวน 31 คน ประกอบด้วย กุมารแพทย์ สูติ-นรีแพทย์ แพทย์เวชปฏิบัติ และพยาบาลวิชาชีพ และทารกแรกเกิดที่สงสัยติดเชื้อ จำนวน 55 ราย เครื่องมือวิจัย ได้แก่ แนวทางการดูแลรักษาภาวะติดเชื้อระยะแรกของทารก แบบประเมินความรู้ แบบสัมภาษณ์เชิงลึก แบบวัดความพึงพอใจ และแบบบันทึกข้อมูลเวชระเบียน วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณนา Paired t-test และวิเคราะห์เชิงเนื้อหา

ผลการวิจัย: ได้แนวทางการดูแลรักษาภาวะติดเชื้อระยะแรกของทารกแรกเกิด คือ แนวทางปฏิบัติการประเมินทารกแรกเกิดที่ติดเชื้อสำหรับพยาบาลวิชาชีพและสำหรับแพทย์ ภายหลังการใช้แนวทางการดูแลรักษาฯ พบว่าพยาบาลวิชาชีพและแพทย์ผู้เกี่ยวข้องมีความรู้สูงกว่าก่อนการพัฒนาอย่างมีนัยสำคัญ (p < .001) และมีคะแนนความพึงพอใจ
ต่อการใช้แนวทางการดูแลรักษาฯ อยู่ในระดับสูงมาก (M = 4.74, SD = .43) และสำหรับทารก พบว่า ทารกมีอัตราการติดเชื้อลดลงจาก ร้อยละ 31.30 เป็นร้อยละ 20 ค่าเฉลี่ยจำนวนวันนอนลดลงจาก 7.90 วัน เป็น 6.80 วัน และค่ารักษาเฉลี่ยลดลงจาก 25,989.43 บาท/ราย เป็น 22,901.73 บาท/ราย

สรุปผล: สามารถใช้แนวทางการดูแลรักษาฯ ที่ได้รับการพัฒนาใหม่ ในการเฝ้าระวังดูแลผู้ป่วยที่สงสัยติดเชื้อได้

ข้อเสนอแนะ: สามารถนำผลการวิจัยไปใช้ในการพัฒนาสมรรถนะพยาบาลและแพทย์หมุนเวียนและแนวทางปฏิบัติในหอผู้ป่วยทารกแรกเกิด
