Effect of Influenza Vaccination on Hospitalization in Sakon Nakhon Province, Thailand

Abstract
This study aimed to analyze risk factors for hospitalization and compare the length of hospital stay among influenza patients by conducting a secondary analysis of data from 10,187 patients in Sakon Nakhon Province between 2022 and 2024, Data were analyzed by using descriptive statistics, Multiple logistic regression, and Poisson regression analyses. The results showed that most patients were children aged 6–14 years (45.3%), and the vaccination rate was low at only 15.8%. Vaccination status was not associated with the likelihood of hospitalization (p=0.610). However, Poisson regression analysis revealed that unvaccinated patients had a hospital stay duration 2.07 times longer than vaccinated patients (Adj. IRR = 2.07, 95% CI: 1.89–2.26, p<0.001). Risk factors for hospitalization included having diabetes (AdjOR=1.35, 95% CI: 1.00–1.82, p=0.050), kidney disease (AdjOR=1.88, 95% CI: 1.05–3.36, p=0.033), and having at least one underlying disease (AdjOR=1.48, 95% CI: 1.28–1.71, p=0.001). Factors associated with longer hospitalization included diabetes (AdjIRR=1.30, p<0.001), kidney disease (AdjIRR=1.34, p=0.007), chronic obstructive pulmonary disease (AdjIRR=1.41, p=0.017), and immunocompromised conditions (AdjIRR=1.76, p=0.002). In conclusion, not receiving influenza vaccination is a significant factor prolonging hospital stay, while having underlying diseases increases both the risk of hospitalization and its duration. These findings support policies promoting influenza vaccination, especially among high-risk groups with comorbidities, to reduce disease severity and healthcare system burden.
