การพัฒนาและใช้แบบประเมินผลกระทบของฟัน ที่สะท้อนคุณภาพชีวิตฉบับภาษาไทย เพื่อประเมินการใช้งานฟันเทียมของผู้สูงอายุในโครงการฟันเทียมพระราชทาน

บทคัดย่อ
ผู้สูงอายุที่ใช้งานฟันเทียมภายใต้โครงการฟันเทียมพระราชทาน มีพฤติกรรมการใส่ฟันเทียมในรูปแบบแตกต่างกันไป แต่ยังไม่เคยมีการประเมินผลกระทบต่อคุณภาพชีวิตและการค้นหาปัจจัยที่ส่งผลต่อผลกระทบของฟันเทียมต่อคุณภาพชีวิต ดังนั้นการศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาและทดสอบความเที่ยงตรงและความเชื่อมั่นของแบบประเมินผลกระทบของฟันที่สะท้อนคุณภาพชีวิต ฉบับภาษาไทย จากนั้นจึงนำแบบประเมินไปใช้เพื่อประเมินผลกระทบของฟันเทียมต่อคุณภาพชีวิต และศึกษาปัจจัยที่สัมพันธ์กับผลกระทบของฟันเทียมต่อคุณภาพชีวิตด้านต่าง ๆ โดยทำการเก็บข้อมูลในผู้สูงอายุที่ได้รับการใส่ฟันเทียม จำนวน 215 คน ในอำเภอปากชม จังหวัดเลย โดยวิธีการสัมภาษณ์และตรวจสภาวะสุขภาพช่องปาก ผลการพัฒนาและทดสอบความเที่ยงตรงและความเชื่อมั่นของแบบประเมินพบว่า แบบประเมินฉบับภาษาไทยมีค่าความเที่ยงตรงเชิงเนื้อหาทั้งฉบับเท่ากับ 0.96 ความเที่ยงตรงตามโครงสร้างพบว่าทุกข้อมีความเที่ยงตรงเชิงโครงสร้างที่ดี (P<0.05) ยกเว้นข้อคำถามเรื่องการจูบหรือหอมแก้ม (P=0.083) และความสอดคล้องภายในมีค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.970 และผลการนำแบบประเมินที่พัฒนาแล้วไปใช้พบว่า ฟันเทียมส่งผลดีต่อการยิ้มและเป็นสัดส่วนมากที่สุด (ร้อยละ 78.1) ส่งผลเสียต่อการเคี้ยวและการกัดเป็นสัดส่วนมากที่สุด (ร้อยละ 29.7) และไม่ส่งผลกระทบใด ๆ ต่อการจูบหรือหอมแก้ม (ร้อยละ 47.6) อายุ ระดับการศึกษา รายได้ต่อเดือน ลักษณะบุคลิกภาพ การมีหรือไม่มีคู่สบฟันธรรมชาติ ประเภทฟันเทียมมีความสัมพันธ์กับผลกระทบของฟันเทียมต่อคุณภาพชีวิตในบางมิติ (P<0.05) และพฤติกรรมการใส่ฟันเทียมสัมพันธ์กับผลกระทบของฟันเทียมต่อคุณภาพชีวิตในทุกมิติ (P<0.001) ผลการศึกษานี้ สรุปได้ว่าแบบประเมินผลกระทบของฟันที่สะท้อนคุณภาพชีวิต ฉบับภาษาไทย ที่ถูกพัฒนาขึ้น มีความเที่ยงตรงและความเชื่อมั่นอยู่ในระดับที่ดีสามารถนำไปใช้กับประชากรไทยได้ และเมื่อนำแบบประเมินไปใช้สามารถอธิบายผลกระทบลักษณะต่าง ๆ ของฟันเทียมต่อคุณภาพชีวิตในมิติต่าง ๆ และปัจจัยข้อมูลทั่วไปมีความสัมพันธ์กับผลกระทบของฟันเทียมต่อคุณภาพชีวิต
