Perception of medication errors in medication use process: A case study of hemodialysis unit in a hospital

Abstract
This survey research aims to study perception of medication errors in medication use process, and factors associated with health care providers’perception in medication errors. This research designs is cross-sectional descriptive study, by interviewing with physicians, pharmacists and nurses. Research instruments is questionnaire, that will be used for data collection about perception of medication use process, by interviewing all of 42 health care providers in hemodialysis unit. Data is analyzed for percentage, frequency, mean, standard deviation, and Pearson’s Correlation Coefficient for testing of association. The result revealed health care providers’ perception in medication errors at the lower level. Perception of health care information systems (HIS) showed moderate associated with health care providers’ perception in medication errors statistically significant at the level of 0.01. The recommendation to administration for develop hospital information systems (HIS), available connection of both system for decrease medication errors in hemodialysis unit.
