ลักษณะปัญหาด้านพฤติกรรม อารมณ์และจิตใจ ในผู้ป่วยโรคสมองเสื่อมจากโรคหลอดเลือดสมอง

บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาความชุกและแนวโน้มการเปลี่ยนแปลงของพฤติกรรมอารมณ์และจิตใจ (Behavioral and Psychological Symptoms of Dementia: BPSD) ในผู้ป่วยโรคสมองเสื่อม จากโรคหลอดเลือดสมอง เมื่อติดตามอาการ 24 สัปดาห์ และ ปัจจัยที่เกี่ยวข้องกับ BPSD
วิธีการศึกษา เป็นการศึกษาเชิงพรรณนาแบบเก็บข้อมูลย้อนหลัง (retrospective descriptive study) รวบรวมข้อมูลจากบันทึกรายงานทางการแพทย์ของผู้ป่วยโรคสมองเสื่อมจากโรคหลอดเลือดสมองที่มารับ การรักษาที่แผนกผู้ป่วยนอกจิตเวช โรงพยาบาลศรีนครินทร์ในช่วงปี พ.ศ. 2557-2561 จำนวน 54 ราย เก็บข้อมูลทั่วไป ข้อมูลการเจ็บป่วย และประเมินปัญหาพฤติกรรมอารมณ์และจิตใจในผู้ป่วย โรคสมองเสื่อมด้วย Neuropsychiatric Inventory Questionnaire Thai version (NPI-Q Thai) วิเคราะห์ ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติเชิงวิเคราะห์ โดยใช้การวิเคราะห์สหสัมพันธ์ (correlation analysis)
ผลการศึกษา ผู้ป่วยโรคสมองเสื่อมจากโรคหลอดเลือดสมองทั้งหมด 54 คน ส่วนใหญ่เป็นผู้ชาย 29 คน (ร้อยละ 53.7) อายุเฉลี่ย 71.9 ± 8 ปี ความรุนแรงของโรคตาม clinical global impression scale (CGI-S) ประมาณ 1 ใน 3 ของผู้ป่วยอยู่ในระดับป่วยเล็กน้อย (ร้อยละ 27.8-38.9) ความชุกของ BPSD อยู่ที่ร้อยละ 87 และ 83.3 ในสัปดาห์ที่ 1 และ 24 ตามลำดับ อาการ BPSD ที่พบบ่อยทั้ง 2 ครั้งคือ อารมณ์เฉยเมย (ร้อยละ 50-55.6) อารมณ์หงุดหงิด (ร้อยละ 31.5-38.9) พฤติกรรมกระวนกระวาย ก้าวร้าว (ร้อยละ 22.2-38.9) อารมณ์วิตกกังวล (ร้อยละ 25.9-31.5) ปัญหาการนอน (ร้อยละ 29.6) และปัญหาการกิน (ร้อยละ 24.1-27.8) อาการที่พบน้อยที่สุดคืออารมณ์ดีกว่าปกติ (ร้อยละ 3.7-9.3) ไม่พบปัจจัยที่มีสหสัมพันธ์กับการเกิด BPSD อย่างมีนัยสำคัญทางสถิติ (p>0.05)
สรุป ความชุกของ BPSD ในผู้ป่วยโรคสมองเสื่อมจากโรคหลอดเลือดสมองพบได้สูงถึงร้อยละ 87 และเมื่อ ติดตามไป 24 สัปดาห์ ความชุกเปลี่ยนแปลงเพียงเล็กน้อย ลักษณะอาการ BPSD ที่พบบ่อยคล้ายคลึงกัน ทั้ง 2 ช่วงเวลา โดยไม่พบปัจจัยที่มีสหสัมพันธ์กับการเกิด BPSD อย่างมีนัยสำคัญทางสถิติ ดังนั้นควร ค้นหาอาการ BPSD ในผู้ป่วยกลุ่มนี้และพัฒนาแนวทางการให้ความช่วยเหลือผู้ป่วย ผู้ดูแลและครอบครัว
