Abstract
An action research aimed to study situation, develop and study the outcomes of a nursing service model for the management of disturbances in the elderly under cataract surgery. The purposived samples consisted of 40 nurses and 84 cataract surgery elderly. The research was conducted in 3 phases; 1) study the situation 2) develop and test the model and 3) evaluate. Research tools were the developed model and the model manual. Data collection tools were 1) a competency assessment form, 2) a disturbance supervision evaluation form, and 3) an opinion assessment form, 4) the questionnaire on the satisfaction of nurses in the second group on the model and 5) the clinical outcome record form. The tool quality values were 0.72, 0.82, 0.95, 0.96 and 0.90 respectively. Descriptive statistics, the t-test, the Mann-Whitney U test, Spearman Rank Correlation, and content analysis were used to analyze the data.
The results revealed that the nursing service model for symptom distress management in older persons under cataract surgery consisted of 1) driving policies 2) nursing management managers and supervising nurses 3) nursing practice policies and procedures, as well as user manuals 4) clinical supervision strategy 5) videos, brochures, and handbooks for older persons and caregivers 6) plan for nursing competency development. The results showed that all patients in the experimental group had fewer symptom distress and cooperated in surgery and treatment, satisfaction was higher in the experimental group than in the control group (p<.05), and the rate of postponement was lower. The nurses reported that the use of the model was feasible, appropriate, correct, helpful; competency in management was higher than before (p<.05); and skills in assessment and management was 86.55%. The clinical supervision showed that the nurses used the model at 87.44%. The nurses were satisfied with the model and accepted at a high level.
