Development of A to I Nursing Care Model for Staff nurses on Self-care agency in Breast Cancer Surgery Patients at The National Cancer Institute

This research and development study aimed to develop an A to I nursing model for staff nurses and study the outcomes of using this model in caring the breast cancer patients undergoing surgery. The purposive samples consisted of 25 professional nurses and 57 breast cancer patients undergoing surgery. This study consisted of 3 phases: (1) the patient care situational analysis; (2) model development; and (3) evaluation of using the model. The research instruments composed of: (1) personal information questionnaires; (2) the A to I nursing model for staff nurses; (3) the knowledge test on nursing care of breast cancer patients undergoing one-day surgery; (4) the evaluation form on nursing performance; (5) the patient self-care agency questionnaire; and (6) the interview and group discussion recording form. The content validity was examined by five experts, yielding CVI of .88-1.0, and Cronbach's alpha coefficients ​​of the 3rd, 4th, and 5th instruments were .83, .85, and .90, respectively. Data were analyzed using descriptive statistics, Wilcoxson signed rank test, and content analysis.


The results found that the A to I nursing model for staff nurses consisted of 3 phases: (1) preparation phase in 4 areas: 1.1 structure, 1.2 services, 1.3 staff nurses, and 1.4 patient care standard; (2) patient care phase; and (3) end of the patient care phase. The results of using the model revealed that after using the model, the patients had significantly higher overall and sub-dimensional self-care ability than before using the model (p < .001). Knowledge of the staff nurses increased by 18.8 percent after received the training.
