The Development of a Clinical Supervision Model for Prevention of Catheter-Associated Urinary Tract Infection Patients

Abstract


             This research and development approach to create and evaluate a clinical supervision model aimed at reducing catheter-associated urinary tract infections (CAUTI). Participants were categorized into three groups: 1) Supervisors (18 head nurses), 2) Supervisees (191 professional nurses), and 3) Patients (604 patients with urinary catheters, purposively divided into two groups of 302). The study utilized various tools, including a knowledge test on clinical supervision (KR-20 = 0.86) and CAUTI prevention (KR-20 = 0.88), compliance records (interrater reliability = 1), nursing outcome records (CVI = 0.84), and supervisor feedback forms (CVI = 0.86).


             Result: The results were shown as following. 1.The model consisted of six components: 1) Disease-specific clinical supervision policies, 2) Clearly defined roles of clinical supervisors, 3) Supervisor knowledge on supervision and CAUTI prevention, 4) A supervision manual, 5) A CAUTI Bundle-based supervision plan, and 6) Context-specific supervision activities. 2. The outcomes were as follows: 1) Patient Outcomes: The CAUTI incidence decreased from 27.2% (82 patients) to 4% (12 patients) (p < .001). 2) Supervisor Outcomes: Knowledge scores on clinical supervision increased from 21.28 to 24.28 (p < .05), and CAUTI prevention knowledge scores rose from 19.33 to 25.39 (p < .05). Satisfaction with the model was rated highly (x̄ = 4.22, SD = 0.43). 3) Supervisee Outcomes: CAUTI prevention knowledge scores improved from 15.94 to 23.25 (p < .05). Supervisees followed key nursing protocols, including correct catheter caring, hand hygiene, aseptic techniques, and closed systems maintaining.
