In Nonthaburi City Municipality, around 40% of diabetes and hypertension patients do not have their control disease, despite the readiness to lead in employing independent nurse freelances for home visit to enhance traditional care services.

The results of the management system and the employment of independent nurse freelances for home visit for diabetes and hypertension patients care show a decrease blood sugar level, as well as a reduction in average upper (SBP) and lower (DBP) blood pressure levels. The workload of community health nurse has decreased, while nurse freelances have gained confidence and flexibility in managing the health problems of diabetes and hypertension patients whose conditions are uncontrolled at home.
