Effects of Nursing Case Management in Elderly Chronic eye uncontrolled Blood sugar and Blood pressure

Abstract


The objective of this academic article was to study the effects of case-based nursing management in conjunction with an ophthalmology support system and education among older adults with a high risk of diabetic retinopathy. This qualitative study involved an in-depth analysis of three case studies of elderly patients with diabetic retinopathy who were at high risk due to uncontrolled blood sugar and blood pressure levels. The study took place at the M Floor, Lertpracharak Building, Mettapracharak Hospital (Wat Rai Khing). The nursing management process consisted of four steps: (1) problem assessment and care planning, (2) coordination, (3) monitoring and supervision, and (4) outcome evaluation. Patients were followed up three times at scheduled appointments and continuous nursing care was provided at home. The results of the study revealed that in case study 1, a 64-year-old male patient, blood sugar levels decreased from 169 mg/dl to 149 mg/dl, with HbA1c remaining at 8%, and blood pressure readings of 177/82mmHg, 161/76mmHg, and 158/72mmHg, respectively. In case study 2, a 61-year-old female patient's blood sugar levels decreased from 190 mg/dl to 160 mg/dl, with HbA1c dropping from 9.6% to 8%, and blood pressure levels of 146/57mmHg, 143/56mmHg, and 158/72mmHg, respectively. In case study 3, a 70-year-old male patient's blood sugar levels reduced from 189 mg/dl to 134 mg/dl, while HbA1c remained at 7.5%. Blood pressure levels were 170/64mmHg, 170/70mmHg, and 174/70mmHg, respectively. These findings indicate that case-based nurse management was effective in controlling both blood sugar and blood pressure levels.
