Development of Learning Program for Knowledge and Abilities Construction for Scrub Nurses in Nursing Care for Patients Who Received Hip Fracture Surgery

Abstract
The purposes of this research and development design were develop and evaluate results of the learning program of scrub nurses using adult learning concept. Purposive sampling method was use to recruit 35 scrub nurses and 105 patients, who received hip fracture surgery. The study was divided into 4 steps: (1) situation analysis (2) program development (3) program implementation (4) program evaluation. Research instruments include handbook for scrub nurses and a training schedule. Data collecting tools consisted of (1) nursing knowledge assessment form (2) nursing skill assessment form (3) scrub nurses’ satisfaction questionnaire (4) patient satisfaction questionnaire (5) surgical time record form (6) total blood loss record form (7) the incidence form of postponed surgery (8) length of stay in the hospital record form. The quality of those tools were assessed using reliability yielding the number of 0.71, 0.94, 0.96, 0.87, 1, 1, 1 and 1 respectively. Data were analyzed using descriptive statistic, t-test and Mann-Whitney U-Test.
The findings revealed that the learning program for knowledge and abilities construction for scrub nurses in nursing care for patient who received hip fracture surgery consisted of 4 activities: (1) creating the participants’ self-concept (2) enhancing their experience (3) preparing to learn (4) Create the learning guidelines. The outcomes of using this learning program included the knowledge score after the program implementation was significant higher than before participated in the program (p < .05). All of the scrub nurses also were able to perform all essential skills. The scrub nurses and the patients were satisfied with the program at a high level. Length of surgery time, amount of blood loss and the number of hospital stay in the experimental group were significantly less than the control group (p<.05). There was no incidence of postponement the surgery due to the nursing team.
