The Development of Clinical Nursing Practice Guideline for Weaning from Mechanical Ventilationin Patients with Brain Surgery, Surgical Intensive Care Unit, Loei Hospital

Abstract
This Research and Development aims to develop the Clinical Nursing Practice Guideline (CNPG) for weaning from mechanical ventilation (MV) in patients with brain surgery and evaluate the outcome of CNPG. The operation consisted of four phases; 1) Evidence triggered phase, 2) Evidence supported phase, 3) Evidence observed phase, and 4) Evidence-based phase. The purposive sample selection included 1) The twelve patients with brain surgery and 2) The sixteen registered nurses working in the Surgical Intensive Care Unit (SICU), Loei hospital. The study was conducted from December 2020 to April 2021.The descriptive statistics, percentage, and mean were used to analyse the quantitative data. Content analysis was used to analyse the qualitative data.
The finding of evidence triggered phase was found the current CNPG for weaning from MV was unsuitable in patients with brain surgery led toa search for evidence-based to develop the CNPG for weaning from MV in patients with brain surgery in the evidence supported phase. The evidence observed phase found that five patients with brain surgery were a success in weaning from MV at 100%, and the result of evidence-based phase was found that twelve patients using the CNPG for weaning from MV had a success rate in weaning at 100%. The average time of using MV was6.7 days (SD + 9.1), and the registered nurses had a 100% satisfied rate on the CNPG for weaning from MV.
