Focus charting nursing records are the documenting guidelines that reflect nursing practice on patient-centered care.  This model represented the nursing process using assessment, intervention and evaluation to support the nursing focus-list statements by documenting key points, goals or expected outcomes. Therefore, focus charting nursing records are important tools for the nursing students to enhance understanding the details of practical training and applying the nursing process to assess the patient's health status and guide for effective nursing documentation according to the nursing process. The preparation process between the instructors and the students should be organized to create issues for understanding of the nursing students. The instructors prepare the students on readiness of practical training by communication and preparation in specific matters for effective practice. The practical training manual that contains details of focus charting nursing records, observes emotional reactions, and psychological care are important issues that helps the students acquiring self-confidence and understanding of nursing practice including documentation the nursing records accurately and efficiently.
