Abstract

Postpartum hemorrhage is a serious obstetric complication that causes hemorrhagic shock leading to maternal death. Caesarean section is a predisposing factor of postpartum hemorrhage both in the present and subsequent pregnancy. Nurses play an important role in caring for and preventing postpartum hemorrhage.

This paper presents the use of the nursing process in the case study of the second pregnancy mother, initially diagnosed with pregnancy 38+1 weeks. She had a history of previous cesarean section and was set to OR for cesarean section and tubal ligation. She developed shock from postpartum hemorrhage due to uterine atony after cesarean section. The physician prescribes the uterotonic agents to improve uterine contraction, fluids and blood transfusion; but she did not recover and was transferred to the intensive care unit and scheduled for subtotal hysterectomy. She was admitted at the intensive care unit for 2 days. She recovered well without shock, normal vital signs and endotracheal tube removal and was transferred to the postpartum unit. She performed self-care and took care of the baby very well. No complication was found and no problem with breast feeding. She was discharged from the hospital with the baby with a total hospitalization of 7 days. Thus, this case study showed that the nurses used the nursing process to care for mothers after caesarean section, ranging from risk factors assessment and close monitoring, planning, nursing care and appropriate reassessment as well as coordinating with the multidisciplinary team to effectively care for the patients. Therefore, the mother after giving birth was safe.
