Development of the Nursing Care Model to Prevent Crisis in Pregnant Women with Hypertensive Disorders at Sakon Nakhon Hospital

Abstract


The objectives of this research and development study were to develop and examine the effects of the nursing care model to prevent crisis in pregnant women with hypertensive disorders. The study was conducted during July 2018 - September 2020. A sample of 31 registered nurses and 52 pregnant women with hypertensive disorders were recruited. The research instruments consisted of the developed nursing care model to prevent crisis in pregnant women with hypertensive disorders, a nurse competency assessment form, a clinical supervision assessment form, and a satisfaction assessment form. Data were analyzed using frequency, percentage, mean, standard deviation and Chi-square.


The results revealed that the nursing care model to prevent crisis in pregnant women with hypertensive disorders consisted of three components: 1) clinical nursing practice guidelines containing 2 parts: 1.1 assessment of the risks and early warning signs, 1.2 nursing care during antepartum, intrapartum, and postpartum period; 2) ten competencies of nursing care in pregnant women with hypertensive disorders; and 3) clinical supervision. The outcomes of the pregnant women shown the incident rate of severe preeclampsia after implementing the model was significant lower than before implementing the model (p < .05). The incident rate of unplanned ICU transfer and magnesium toxicity was not report. The outcomes of the nurses indicated that knowledge after implementing the model was significant higher than before implementing the model (p < .05). Their skill passed the criteria at 95.28 and they correctly performed nursing care according to the model at 92.82 per cent.
