Development of Nursing Management Model for Receiving and Referral of Myocardial Ischemia Patients at Yasothon Province.

Abstract 


The objectives of this research and development was to develop and to study the results of the nursing management model on receiving and referring patients with myocardial ischemia, Yasothon Province.  A purposive sample of 102 myocardial ischemia patients and 36 professional nurses at Yasothon Hospital and community hospitals of Yasothon Province network, were recruited. The study consisted of 4 phases: (1) preparation and situation analysis (2) develop the nursing administration model (3) model implementation (4) evaluation. The research instruments composed of (1) professional nurse competency development program (2) nursing standards (3) nursing supervision plan and were checked for quality using 5 experts. Data collecting tools included (1) a knowledge test of the nurses, yielding the difficulty level of 0.6 (2) a satisfaction questionnaire of the nurses, yielding Cronbach's alpha coefficient of 0.81 (3) performance assessment according to the nursing standards of the nurses (4) a record form of the nursing quality indicators. Data were analyzed using descriptive statistics, pair t-test, and Chi-square.


        The results showed that the nursing management model on receiving and referring patients with myocardial ischemia, Yasothon Province consisted of professional nurse competency development programs, nursing standards, and nursing supervision. Outcomes of the quality nursing care included (1) door to EKG within 10 minutes (2) streptokinase injection (3) door to needle within 30 minutes (4) onset to needle time within 180 minutes, that significantly increased at the level of .05. The complications on major bleeding due to streptokinase injection was statistically significant decreased at the level of .05. Provider outcomes on knowledge of the nurses revealed that the average score after development was significant higher than before development (Mean before 11.72, Mean after 18.75). The nurses reported their overall satisfaction at the highest level (Mean 4.71) and practice according to the nursing standards at 73.33–93.33 percent.
