บทคัดย่อ


การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลลัพธ์การใช้รูปแบบการดูแลผู้ป่วยบาดเจ็บศีรษะเล็กน้อย กลุ่มตัวอย่าง ได้แก่ ผู้ป่วย 40 คน ผู้ดูแล 40 คน และบุคลากรทีมสหสาขาวิชาชีพ 52 คน การพัฒนามี 4 ขั้นตอน ได้แก่ (1) การวิเคราะห์สถานการณ์ (2) การพัฒนารูปแบบฯ (3) การใช้รูปแบบ (4) การประเมินผล เครื่องมือที่ใช้ในการดำเนินการวิจัย ได้แก่ รูปแบบการดูแลผู้ป่วยบาดเจ็บศีรษะเล็กน้อย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบสัมภาษณ์ แบบตรวจสอบเวชระเบียน แบบทดสอบความรู้ของผู้ป่วยและของพยาบาล แบบประเมินความสามารถของผู้ดูแล แบบประเมินทักษะของพยาบาล แบบประเมินความพึงพอใจของผู้ป่วยและผู้ดูแล พยาบาลและทีมสหสาขาวิชาชีพ ค่าความเชื่อมั่นเท่ากับ .79, .80, .90, .89, .92 และ .95 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบ paired t-test และวิเคราะห์เชิงเนื้อหา


ผลการวิจัยพบว่า รูปแบบการดูแลผู้ป่วยบาดเจ็บศีรษะเล็กน้อย ประกอบด้วย 6 องค์ประกอบ (1) แผนการดูแลผู้ป่วยบาดเจ็บศีรษะเล็กน้อย (2) พยาบาลผู้จัดการรายกรณี (3) การใช้แนวปฏิบัติการพยาบาล (4) การมีส่วนร่วมของบุคคลในครอบครัว (5) การวางแผนจำหน่าย (6) การติดตามเยี่ยมทางโทรศัพท์ หลังนำรูปแบบไปทดลองใช้ พบการกลับมารักษาซ้ำภายใน 72 ชั่วโมงจำนวน 1 ราย ไม่พบผู้ป่วยเกิดภาวะเลือดออกในสมอง ผู้ดูแลมีความรู้สูงขึ้นอย่างมีนัยสำคัญทางสถิติ ($p<.01$) ในด้านความสามารถในการดูแลผู้ป่วยพบว่ามีความสามารถอยู่ในระดับสูง (Mean 18.20, SD 0.96) ผู้ป่วยและผู้ดูแลมีความพอใจในระดับมากที่สุด (Mean 4.88, SD .47) พยาบาลมีความรู้ในการดูแลผู้ป่วยสูงขึ้น ($p<.01$) พยาบาลและทีมสหสาขาวิชาชีพมีความพึงพอใจต่อการใช้รูปแบบในระดับมากที่สุด (Mean 4.75, SD .58)
