บทคัดย่อ
การวิจัยและพัฒนาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลลัพธ์แนวปฏิบัติการพยาบาลหย่าเครื่องช่วยหายใจในผู้ป่วยเด็ก ห้องผู้ป่วยหนักกุมารเวชกรรม โรงพยาบาลสระบุรี วิธีการดำเนินงานมี 4 ขั้นตอน คือ (1) วิเคราะห์สถานการณ์ (2) พัฒนาแนวปฏิบัติ (3) นำแนวปฏิบัติไปใช้ (4) ประเมินผล กลุ่มตัวอย่างคัดเลือกแบบเฉพาะเจาะจง ประกอบด้วยพยาบาลวิชาชีพ 12 คน ผู้ป่วยเด็ก 30 คน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แนวปฏิบัติและคู่มือการพยาบาลหย่าเครื่องช่วยหายใจในผู้ป่วยเด็ก เครื่องมือรวบรวมข้อมูล ได้แก่ 1) แบบทดสอบความรู้ตามแนวปฏิบัติ 2) แบบประเมินทักษะตามแนวปฏิบัติ 3) แบบสอบถามความคิดเห็นของพยาบาลต่อแนวปฏิบัติ 4) แบบสอบถามความพึงพอใจของผู้ดูแล และ 5) แบบบันทึกผลลัพธ์ทางคลินิกจากการใช้แนวปฏิบัติ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน
ผลการวิจัย พบว่า แนวปฏิบัติ ประกอบด้วย (1) ระยะ Pre-weaning การประเมินความพร้อมในการหายใจเอง (2) ระยะ Weaning การทดสอบการหายใจเองและการหย่าเครื่องช่วยหายใจ (3) ระยะ Weaning outcome การพยาบาลขณะถอดท่อช่วยหายใจและการประเมินความสำเร็จ ผลลัพธ์มีดังนี้ 1) ด้านผู้ให้บริการพบว่าพยาบาลมีความรู้ก่อนและหลังการใช้แนวปฏิบัติแตกต่างกันอย่างมีนัยสำคัญ (P < .05) มีทักษะในการปฏิบัติตามแนวปฏิบัติครบ 57 กิจกรรมร้อยละ 92 ความคิดเห็นของพยาบาลต่อแนวปฏิบัติอยู่ในระดับมากที่สุด (X̄ = 4.93, S.D. = .24) 2) ด้านผู้ใช้บริการพบว่าความพึงพอใจของผู้ดูแลอยู่ในระดับมาก (X̄ = 4.29, S.D. = .51) อัตราการหย่าเครื่องช่วยหายใจสำเร็จเพิ่มจากร้อยละ 69.23 เป็น 96.67 อัตราการใส่ท่อช่วยหายใจซ้ำ (Re-intubation) ลดจากร้อยละ 6.67 เป็น 3.33 อัตราการเกิดปอดอักเสบที่สัมพันธ์กับการใส่เครื่องช่วยหายใจลดจาก 9.63 เป็น 3.33 ครั้ง/1,000 วันใส่เครื่องช่วยหายใจ จำนวนวันใส่เครื่องช่วยหายใจลดจาก 13.77 วัน เป็น 6.26 วัน ค่ารักษาพยาบาลลดจาก 99,821 บาท เป็น 71,199 บาท และจำนวนวันในการนอนรักษาเฉลี่ยลดจาก 15.24 วัน เป็น 10.52 วัน
สรุปการนำแนวปฏิบัติมาใช้ในการพัฒนาการดูแลทำให้เกิดผลลัพธ์ที่ดีและสามารถเพิ่มคุณภาพการดูแลผู้ป่วยเด็กได้อย่างมีประสิทธิภาพ
