บทคัดย่อ
การวิจัยดำเนินการ (Implementation research) นี้มีวัตถุประสงค์ เพื่อพัฒนาและศึกษาประสิทธิผลของการใช้ แนวปฏิบัติการป้องกันพลัดตกหกล้ม วัดผลลัพธ์ด้านการพยาบาล ได้แก่ ความรู้ การปฏิบัติ และความพึงพอใจก่อนและหลังใช้แนวปฏิบัติ ผลลัพธ์ด้านผู้ป่วย คือ อุบัติการณ์พลัดตกหกล้ม กลุ่มตัวอย่าง คือ ผู้สูงอายุหลังได้รับการผ่าตัดหัวใจแบบเปิด จำนวน 30 ราย และพยาบาลวิชาชีพในหอผู้ป่วย จำนวน 21 ราย เครื่องมือที่ใช้ ได้แก่ แนวปฏิบัติการป้องกันพลัดตกหกล้มในผู้สูงอายุหลังผ่าตัดหัวใจแบบเปิด แบบบันทึกข้อมูลส่วนบุคคลของผู้ป่วย แบบบันทึกข้อมูลด้านสุขภาพ แบบบันทึกการเกิดเหตุการณ์พลัดตกหกล้ม แบบสอบถามข้อมูลส่วนบุคคลของพยาบาล ตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน และตรวจสอบความเชื่อมั่นของแบบสังเกตการณ์ปฏิบัติตามแนวทางป้องกันพลัดตกหกล้ม แบบทดสอบความรู้ในการป้องกันพลัดตกหกล้มของผู้สูงอายุหลังผ่าตัดหัวใจแบบเปิด และแบบประเมินความพึงพอใจของพยาบาลวิชาชีพ ได้ค่า CVI เท่ากับ 0.88, 0.60 และ 0.89 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย one sample t-test และ Z- test
ผลการวิจัย: พบว่า หลังใช้แนวปฏิบัติการป้องกันพลัดตกหกล้ม พยาบาลปฏิบัติตามแนวปฏิบัติฯ มากกว่าร้อยละ 80 พยาบาลมีความรู้ในการป้องกันพลัดตกหกล้มสูงกว่าก่อนใช้แนวปฏิบัติ อย่างมีนัยสำคัญทางสถิติ (p < .001) ความพึงพอใจจากการนำแนวปฏิบัติไปใช้ของพยาบาลสูงกว่าเกณฑ์ ร้อยละ 80 อย่างมีนัยสำคัญทางสถิติ (p < .05) และอุบัติการณ์พลัดตกหกล้มลดลงกว่าก่อนใช้แนวปฏิบัติฯ แต่ไม่มีนัยสำคัญทางสถิติ (p > .05)
