บทคัดย่อ
การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมนี้ มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลลัพธ์การใช้แนวทางการเฝ้าระวังผู้ป่วยอาการทรุดลงระหว่างรอตรวจ ดำเนินการ 2 ระยะ คือ (1) ศึกษาสถานการณ์ และพัฒนาแนวทางฯ กลุ่มตัวอย่าง และ (2) นำไปใช้และประเมินผลลัพธ์ กลุ่มตัวอย่างในระยะที่ 1 คือ แพทย์ และพยาบาล 7 คน เก็บข้อมูลโดยใช้แนวคำถามสนทนากลุ่มแบบกึ่งมีโครงสร้าง กลุ่มตัวอย่างในระยะที่ 2 คือ พยาบาลประจำห้องตรวจอายุรกรรม 26 คน และผู้ป่วยที่มารับบริการในระหว่างทำการศึกษา 11,252 คน เครื่องมือวิจัย ประกอบด้วย (1) แนวทางการเฝ้าระวังผู้ป่วยอาการทรุดลงระหว่างรอตรวจ, (2) Sakon Nakhon Hospital Modified Early Warning Signs (S-MEWS), (3) Search Out Severity (SOS), และ (4) MOPH ER Triage ร่วมกับอาการของผู้ป่วย เครื่องมือเก็บรวบรวมข้อมูล ได้แก่ (1) แบบสังเกตการปฏิบัติงาน และ (2) แบบสอบถามความพึงพอใจ วิเคราะห์ข้อมูลด้วยร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ Independent t-test
ผลการวิจัย พบว่า แนวทางการเฝ้าระวังผู้ป่วยอาการทรุดลงระหว่างรอตรวจที่ห้องตรวจอายุรกรรม ประกอบด้วย (1) แนวทางการคัดแยกความเร่งด่วนของผู้ป่วยจำแนกตามสี และ (2) การพยาบาลตามกลุ่มผู้ป่วย ผลลัพธ์การใช้แนวทางฯ พบว่า ส่วนใหญ่เป็นผู้ป่วยไม่รีบด่วน ร้อยละ 89.31 กลุ่มผู้ป่วยรีบด่วนได้รับการพยาบาลจนอาการดีขึ้นทุกราย กลุ่มผู้ป่วยฉุกเฉิน ทุกคนได้รับการส่งต่อและดูแลต่อในแผนกอุบัติเหตุฉุกเฉิน หลังการใช้แนวทางฯ พยาบาลสามารถคัดแยกผู้ป่วยตามระดับความเร่งด่วนได้ถูกต้อง และสามารถปฏิบัติตามแนวทางฯ ได้ถูกต้องมากกว่าก่อนการใช้แนวทางฯ อย่างมีนัยสำคัญทางสถิติ (p < .05) และมีความพึงพอใจในระดับมาก (Mean 2.79, SD 0.47) และไม่พบอุบัติการณ์การเสียชีวิตของผู้ป่วยระหว่างรอตรวจ
