บทคัดย่อ


การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบแผนการจำหน่ายผู้ป่วยสมองบาดเจ็บไม่รุนแรง และศึกษาผลลัพธ์ของการใช้รูปแบบแผนการจำหน่ายผู้ป่วยสมองบาดเจ็บไม่รุนแรง กลุ่มตัวอย่าง คือ 1) ผู้ป่วยสมองบาดเจ็บไม่รุนแรงและผู้ดูแล จำนวน 120 คน 2) พยาบาลวิชาชีพ จำนวน 42 คน ดำเนินวิจัยตาม PDCA ดังนี้ 1) ศึกษาปัญหาและจัดทำร่างรูปแบบแผนการจำหน่ายผู้ป่วย 2) นำร่างรูปแบบที่ได้สู่การปฏิบัติ 3) ตรวจสอบ กำกับติดตาม 4) นำผลการประเมินมาปรับปรุงอย่างต่อเนื่อง เครื่องมือวิจัย คือ 1) คู่มือรูปแบบแผนการจำหน่ายผู้ป่วย 2) แผ่นพับคำแนะนำและวีดีทัศน์ เรื่อง การดูแลผู้ป่วยสมองบาดเจ็บไม่รุนแรง 3) แบบบันทึกการติดตามอาการและการกลับมาตรวจรักษาซ้ำ 4) แบบสังเกตการปฏิบัติตามรูปแบบแผนการจำหน่าย 5) แบบทดสอบความเข้าใจของผู้ดูแล 6) แบบสอบถามความพึงพอใจของผู้ป่วย ผู้ดูแล และพยาบาล วิเคราะห์ข้อมูลด้วยค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติ t-test


                   ผลการศึกษา พบว่า รูปแบบแผนการจำหน่ายผู้ป่วยสมองบาดเจ็บไม่รุนแรง ประกอบด้วย 1) การประเมินและเตรียมความพร้อมของผู้ป่วยและผู้ดูแลก่อนกลับบ้าน 2) การให้คำแนะนำผู้ป่วยและผู้ดูแล 3) การติดตามอาการผู้ป่วยหลังจำหน่าย  เปรียบเทียบการกลับมาตรวจรักษาซ้ำภายใน 72 ชั่วโมง ในกลุ่มที่ได้รับบริการพยาบาลตามรูปแบบแผนการจำหน่ายและกลุ่มที่ได้รับบริการพยาบาลตามปกติ พบว่าไม่แตกต่างกัน  ความพึงพอใจของผู้ดูแลที่ได้รับบริการพยาบาลตามรูปแบบแผนการจำหน่ายกับกลุ่มที่ได้รับการพยาบาลตามปกติ ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05  ความพึงพอใจของผู้ป่วยและพยาบาลในกลุ่มทดลองสูงกว่ากลุ่มควบคุมแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
