การถอดบทเรียนการระดมสรรพกำลังด้านสาธารณสุขในสถานการณ์การระบาดโควิด-19 ของโรงพยาบาลในกรุงเทพมหานครและปริมณฑล

บทคัดย่อ
การศึกษาการระดมสรรพกำลังด้านสาธารณสุขในสถานการณ์การระบาดโรคโควิด 19 มีวัตถุประสงค์เพื่อพัฒนาข้อเสนอเชิงนโยบายกรณีเกิดการระบาดซ้ำ โดยใช้กรอบแนวคิดการระดมสรรพกำลังในภาวะฉุกเฉิน 4S: (1) พื้นที่ (2) บุคลากร (3) วัสดุอุปกรณ์ (4) ระบบ ศึกษา 3 ระยะ (1) เตรียมความพร้อม (2) เผชิญการระบาด (3) ฟื้นฟูสู่ภาวะปกติ คัดเลือกผู้ให้ข้อมูลสำคัญแบบเจาะจง จำนวน 20 คน เครื่องมือวิจัยเป็นแบบสัมภาษณ์แบบมีโครงสร้าง เก็บข้อมูลโดยการสนทนากลุ่ม วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา
ผลการวิจัย พบว่า มีการระดมสรรพกำลังในภาวะฉุกเฉินตามหลัก 4S ดังนี้ (1) ระยะเตรียมความพร้อมด้านพื้นที่และบุคลากร ประกอบด้วย ลดการให้บริการอื่น และปรับปรุงให้มีห้องแยกความดันลบ ด้านวัสดุอุปกรณ์ เพิ่มระยะเวลาสำรองวัสดุอุปกรณ์ และรับบริจาค ด้านระบบ นำแผนรองรับไข้หวัดใหญ่ 2009 และโรคทางเดินหายใจตะวันออกกลางมาปรับใช้ (2) ระยะเผชิญการระบาด ด้านพื้นที่ ลดการให้บริการอื่น จัดตั้ง ARI Clinic ปรับปรุงหอผู้ป่วยเป็น Cohort Ward เพิ่มห้องแยกแรงดันลบ และ ทำ Hospitel สำหรับผู้ป่วยกลุ่มสีเขียว ด้านบุคลากร ระดมกำลังแพทย์ พยาบาล และบุคลากรทุกฝ่าย จัดเวรผลัด และเหลื่อมเวลา และฝึกอบรมการควบคุมการติดเชื้อแก่บุคลากรทุกระดับ ด้านวัสดุอุปกรณ์ เพิ่มระยะเวลาสำรองวัสดุอุปกรณ์ รับบริจาค และกำจัดขยะตามหลัก Biosafety ด้านระบบ สั่งการแบบ Single command และบูรณาการทำงานของโรงพยาบาลกับ EOC (3) ฟื้นฟูสู่ภาวะปกติ ด้านพื้นที่และบุคลากร มีแผนดำเนินการคืนพื้นที่ บุคลากร และกลับมาให้บริการตามปกติ ด้านวัสดุอุปกรณ์ จัดทำฐานข้อมูลวัสดุอุปกรณ์เพื่อให้ง่ายต่อการจัดเตรียม ด้านระบบ ทำงานข้ามสายงาน และปฏิบัติตามมาตรการควบคุมป้องกันการติดเชื้อในโรงพยาบาลให้เกิดยั่งยืน
