ปัจจัยที่มีผลต่อการควบคุมระดับน้ำตาลในเลือดในผู้ป่วยโรคเบาหวานชนิดที่ 2

การวิจัยเชิงพรรณนาแบบภาคตัดขวางครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อการควบคุมระดับน้ำตาลในเลือดของผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่มารับบริการ ณ คลินิกเบาหวาน โรงพยาบาลแม่สรวย จังหวัดเชียงราย กลุ่มตัวอย่างคืออาสาสมัครผู้เข้าร่วมโครงการวิจัยที่มีคุณสมบัติตรงตามเกณฑ์ที่กำหนด จำนวน 388 ราย เครื่องมือที่ใช้ในการวิจัยประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความรอบรู้ด้านสุขภาพ แบบสอบถามพฤติกรรมการจัดการตนเองแบบสอบถามคุณภาพการนอนหลับ และแบบสอบถามการสนับสนุนทางสังคม วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติถดถอยโลจิสติกไบนารีแบบตัวแปรเดียวและพหุตัวแปร ผลการวิจัยพบว่า
ปัจจัยที่มีความสัมพันธ์กับการควบคุมระดับน้ำตาลในเลือด ได้แก่ ค่าระดับน้ำตาลในเลือดขณะอดอาหาร (FBS) และคุณภาพการนอนหลับ ต่อมาได้คัดเลือกตัวแปรที่มีค่า p-value < .25 ได้แก่ อายุ เชื้อสาย สถานภาพสมรส อาชีพ การสูบบุหรี่ ระยะเวลาที่เป็นโรคเบาหวาน FBS และคุณภาพการนอนหลับ เข้าสู่การวิเคราะห์ถดถอยโลจิสติกไบนารีแบบพหุตัวแปร ผลการวิเคราะห์พบว่า ผู้ที่มีค่า FBS อยู่ระหว่าง 131 – 160 มก./ดล. มีโอกาสควบคุมระดับน้ำตาลในเลือดไม่ได้มากกว่าผู้ที่มี FBS อยู่ในเกณฑ์ปกติ 2.43 เท่า และผู้ที่มีค่า FBS สูงกว่า 160 มก./ดล. มีโอกาสควบคุมระดับน้ำตาลในเลือดไม่ได้มากกว่าผู้ที่มีค่า FBS ในระดับปกติ 11.40 เท่า สำหรับคุณภาพการนอนหลับ พบว่า ผู้ที่มีคุณภาพการนอนหลับไม่ดี มีโอกาสควบคุมระดับน้ำตาลสะสมในเลือด (HbA1c) ไม่ได้มากกว่าผู้ที่มีคุณภาพการนอนหลับดี 2.87 เท่า
สรุปได้ว่า ค่า FBS และคุณภาพการนอนหลับ เป็นตัวแปรที่มีอิทธิพลต่อการควบคุมระดับน้ำตาลในเลือดของผู้ป่วยเบาหวานชนิดที่ 2 ซึ่งผลการวิจัยนี้สามารถใช้เป็นข้อมูลพื้นฐานในการวางแผนการพยาบาล การออกแบบโปรแกรมการดูแลผู้ป่วย การพัฒนารูปแบบการลดระดับน้ำตาลในเลือดและส่งเสริมคุณภาพชีวิตที่ดียิ่งขึ้นในผู้ป่วยกลุ่มนี้
