Incidence and Factors Associated with Intraoperative Anesthetic Complications among Surgical Patients at Rayong Hospital in Honor of Her Royal Highness Princess Maha Chakri Sirindhorn

Abstract

This ambidirectional cohort study aimed to determine the incidence of intraoperative anesthesia-related complications and identify factors associated with these complications among surgical patients at Princess Maha Chakri Sirindhorn Hospital, Rayong. Data were retrospectively collected from the medical records of 2,400 surgical patients. Descriptive statistics were used to summarize demographic and clinical characteristics. Factors associated with anesthesia-related complications were analyzed using multiple logistic regression, and the results were presented as adjusted odds ratios (Adjusted ORs) with 95% confidence intervals. Statistical significance was set at P-value < 0.05.

The incidence of intraoperative anesthesia-related complications was 20.0%. Hypotension was the most common complication, followed by hypertension and hypoxemia. Multivariable analysis demonstrated that age over 65 years, ASA physical status III or higher, emergency surgery, operative duration exceeding 120 minutes, and general anesthesia were significantly associated with anesthesia-related complications.

These findings suggest that systematic preoperative risk assessment and close intraoperative monitoring of high-risk patients may reduce anesthesia-related complications. The results may be applied to improve perioperative anesthesia care, develop evidence-based clinical practice guidelines, and enhance patient safety during surgery.
