Abstract

This research and development (R&D) study aimed to develop and evaluate the effectiveness of a nursing service model for high-alert drug (HAD) administration in the Intensive Care Unit (ICU), Nursing Mission Group, Mae Sai Hospital, Chiang Rai Province Methods: The study was conducted in four sequential phases: 1) situation analysis, 2) development of a preliminary nursing service model for HAD, 3) implementation and evaluation, and 4) refinement of the final model. The participants included 15 intensive care nurses. The study was conducted between December 2024 and March 2025. The research instruments consisted of: 1) the Nursing Service Model and the "Safe Y-Card" innovation (CVI = 0.92 and 0.94, respectively); 2) assessments for knowledge, practice, and satisfaction (KR-20 = 0.82; Cronbach's alpha = 0.90 and 0.91, respectively); and 3) a medication error reporting form. Data were analyzed using percentage, mean, standard deviation, and paired t-test

The results showed that following the implementation of the finalized nursing service model, the participants' mean scores for knowledge and clinical practice were significantly higher than those before the intervention (P-value < 0.001$ and $p = 0.001$, respectively). No medication errors were reported during the study period, and 100% of participants reported the highest level of satisfaction. The final model integrated standard operating procedures (SOPs) with the "Safe Y-Card" innovation, a color-coding system, and extension line identification to minimize cognitive load and prevent confusion. Furthermore, the model incorporates Y-site compatibility management, which effectively enhances nursing competencies and bolsters systemic safety in the care of critically ill patients.
