การประเมินผลโครงการนำส่งยาโรคไม่ติดต่อเรื้อรังที่บ้านในช่วงการระบาดของโควิด-19 อำเภอศิลาลาด จังหวัดศรีสะเกษ

การวิจัยนี้มีวัตถุประสงค์เพื่อ (1) ประเมินผลโครงการนำส่งยาโรคไม่ติดต่อเรื้อรังที่บ้าน ด้านบริบท ปัจจัยนำเข้า และกระบวนการ ในมุมมองของผู้ให้บริการ (2) เปรียบเทียบคะแนนความเหมาะสมการดำเนินโครงการฯ ของผู้ให้บริการระหว่างในและนอกเขตบริการปฐมภูมิ (Primary Care Unit: PCU) ของโรงพยาบาลศิลาลาด (3) ประเมินผลการดำเนินงานโครงการฯ ด้านการได้รับยาและความพึงพอใจของผู้รับบริการ และ (4) เปรียบเทียบความพึงพอใจของผู้รับบริการ ระหว่างกลุ่มที่ได้รับยาก่อนนัดกับกลุ่มที่ไม่ได้รับยาก่อนนัด และระหว่างในกับนอกเขต PCU ของโรงพยาบาลศิลาลาด จังหวัดศรีสะเกษ
รูปแบบการศึกษาเป็นการศึกษาเชิงสำรวจเพื่อประเมินโครงการนำส่งยาโรคไม่ติดต่อเรื้อรังที่บ้าน กลุ่มตัวอย่างกลุ่มแรก คือ ผู้ให้บริการ ที่เป็นผู้รับผิดชอบงานโรคไม่ติดต่อเรื้อรังของโรงพยาบาลส่งเสริมสุขภาพตำบล (นอกเขต PCU) และในเขตของโรงพยาบาลศิลาลาด 6 ราย เลือกแบบเจาะจง และ อสม./ผู้ดูแล โดยสุ่มอย่างง่าย 44 ราย กลุ่มตัวอย่างที่สอง คือ ผู้รับบริการ ซึ่งเป็นผู้ป่วยโรคเบาหวานและโรคความดันโลหิตสูงที่นำส่งยาที่บ้านในช่วงเดือนมิถุนายน พ.ศ. 2563 ถึงเดือน พฤศจิกายน พ.ศ. 2564 โดยการสุ่มอย่างง่ายจำนวน 100 ราย เก็บข้อมูลระหว่างเดือนพฤษภาคม-มิถุนายน พ.ศ.2565 ด้วยแบบสอบถาม2 ชุด มีความเที่ยง 0.91 และ 0.81 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา การทดสอบ t-test และ Mann Whitney U Test
ผลการวิจัยพบว่า การประเมินผลโครงการฯ ของผู้ให้บริการ มีคะแนนเฉลี่ยรายด้านและโดยรวมอยู่ในระดับมาก โดยคะแนนเฉลี่ยความเหมาะสมการดำเนินโครงการฯ ของผู้ให้บริการในเขต PCU (ด้านบริบท 22.30 ± 2.06 ด้านปัจจัยนำเข้า 12.73 ± 1.74 ด้านกระบวนการ 16.55 ± 2.46 และโดยรวม 51.64 ± 4.74) สูงกว่านอกเขต PCU ของโรงพยาบาลศิลาลาด (ด้านบริบท 19.23 ± 3.20 ด้านปัจจัยนำเข้า 11.23 ± 1.91 ด้านกระบวนการ 14.85 ± 2.57 และโดยรวม 45.31 ± 2.27) เมื่อเปรียบเทียบคะแนนพบว่าด้านบริบท ด้านปัจจัยนำเข้า และโดยรวม แตกต่างกันอย่างมีระดับนัยสำคัญทางสถิติ ที่ระดับ 0.05 (p-value=0.006,0.030 และ 0.009) ส่วนด้านกระบวนการไม่มีความแตกต่างกัน การประเมินผลด้านการได้รับยา พบว่าผู้รับบริการในเขต PCU ได้รับยาก่อนนัดในสัดส่วนมากกว่านอกเขต PCU ของโรงพยาบาลศิลาลาด (ร้อยละ 58.50 และ 33.90 ตามลำดับ) และมีความพึงพอใจโดยรวมอยู่ในระดับมากที่สุด (4.48 ± 0.57) และเปรียบเทียบความพึงพอใจของผู้รับบริการที่ได้รับยาก่อนนัด กับไม่ได้รับยาก่อนนัด และผู้รับบริการในเขต PCU กับนอกเขต PCU โรงพยาบาลศิลาลาด พบว่าคะแนนความพึงพอใจไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ
