บทคัดย่อ

บริบท ภาวะหัวใจล้มเหลวชนิดที่มีการบีบตัวของหัวใจปกติ เป็นภาวะที่พบได้บ่อยในเวชปฏิบัติ ซึ่งมีสาเหตุ และ โรคร่วมต่างๆ มากมาย หนึ่งในสาเหตุที่พบได้บ่อย คือ ภาวะหยุดหายใจขณะหลับจากการอุดกลั้น
วัตถุประสงค์ เพื่อศึกษาความสัมพันธ์ระหว่างภาวะหยุดหายใจขณะหลับจากการอุดกลั้นที่ส่งผลต่อระบบโรค หัวใจและหลอดเลือดจนนำไปสู่ภาวะหัวใจล้มเหลว
วิธีการศีกษา ทบทวนวรรณกรรมที่เกี่ยวข้องโดยการสืบค้นจากหนังสือ เอกสารวิชาการและรายงานผลการวิจัยจากหลายแหล่งข้อมูลที่มีความน่าเชื่อถือ จากนั้นเรียบเรียงข้อมูลและนำเสนอถึงอุบัติการณ์ การวินิจฉัย ตลอดจนการรักษาของภาวะหยุดหายใจขณะหลับจากการอุดกลั้น และภาวะหัวใจล้มเหลวชนิดที่มีการบีบตัวของหัวใจปกติ นอกจากนี้ได้นำเสนอถึงความสัมพันธ์ระหว่างภาวะหยุดหายใจขณะหลับจากการอุดกลั้นที่ส่งผลต่อระบบโรคหัวใจและหลอดเลือด
ผลการศึกษา ภาวะหยุดหายใจขณะหลับจากการอุดกลั้นเป็นสาเหตุหนึ่งที่ส่งผลต่อระบบหัวใจและหลอดเลือดเมื่อผู้ป่วยมีภาวะหยุดหายใจขณะหลับจากการอุดกลั้น จะทำให้มีการเปลี่ยนแปลงความดันในช่องอกและเกิดการหายใจที่ไม่มีประสิทธิภาพ ส่งผลให้เกิดภาวะออกซิเจนในเลือดต่ำชั่วคราว ร่วมกับมีภาวะคาร์บอนไดออกไซด์คั่งในเลือด นอกจากนี้ยังมีการกระตุ้นระบบประสาทซิมพาเทติก และกระบวนการอักเสบของร่างกายจนนำไปสู่การเกิดภาวะหัวใจล้มเหลวชนิดที่มีการบีบตัวของหัวใจปกติ
สรุป กลไกหลักของภาวะหยุดหายใจขณะหลับจากการอุดกลั้นที่ส่งผลต่อระบบหัวใจและหลอดเลือด คือ การกระตุ้นระบบประสาทซิมพาเทติก การเกิดกระบวนการอักเสบของร่างกาย กล้ามเนื้อหัวใจขาดเลือด
ภาวะหัวใจเต้นผิดจังหวะชนิด Atrial fibrillation และการเปลี่ยนแปลงของความดันในช่องอก กลไกดังกล่าวทำให้เกิดภาวะหัวใจล้มเหลวตามมาในที่สุด
