The Effects of Self-Management Program Related to Medication Adherence in Uncontrollable Hypertensive Patients at a Private Clinic in Pathumthani Province

Abstract
The purpose of this quasi-experimental research one group pretest-posttest design was to study the effects of self-management program related to medication adherence in uncontrollable hypertensive patients at a private clinic in Pathumthani Province. The research sample were purposive sampling consisted of 28 patients. Self-management program related to medication adherence in uncontrollable hypertensive patients were applied from Kanfer, Gaelick-Buys. The 8 weeks experiment program consisted of 5 steps 1) self-assessment 2) mutual goal setting 3) knowledge and practice skills enhancement 4) self-monitoring and 5) evaluation. The instruments used to collect data were a set of demographic questionnaires which included personal data, knowledge about drug usage and medication usage behaviors. These questionnaires were validated by the five experts which IOC 1.00, and the reliability (KR-20) were 1.00 and 0.80. The research data were collected from July to August 2017. The data were statistically analyzed according to descriptive statistics and paired t-test.
The results indicated that after received the intervention program, the mean scores of knowledge and medication adherence behaviors were significantly higher than before received the program (p < .50). Whereas blood pressure level was lower than those before received the program (p < .50). The research results confirmed that, the self-management program leads to increase knowledge and medication adherence behaviors and reduces blood pressure level. Therefore, this recommends nurse practitioners should realize the importance of home health care and telephone visit continuingly, and utilized both key strategies in order to medication adherence behaviors monitoring among uncontrollable hypertensive patients, for improved outcomes of controls blood pressure level efficiently
