This quasi-experimental study aimed to examine the effects of clinical nursing practice guidelines on preventing unplanned endotracheal extubation in critically ill medical patients at the Medical Intensive Care Unit, Chaiyaphum Hospital. A total of 88 intubated patients were purposively selected and assigned to the control (n = 44) and experimental groups (n = 44). The control group received routine nursing care, while the experimental group received care guided by the nursing practice guidelines for 5 days. Data collection tools included a nursing record form for preventing unplanned endotracheal extubation in critically ill patients, a nurse compliance observation form, and a nurse satisfaction assessment regarding the use of the nursing practice guideline. The content validity test of the tools obtained a Content Validity Index (CVI) of 0 . 8 9 and the reliability test yielded a Cronbach's alpha coefficient of 0.89. Data were analyzed using the chi-square test.
Results revealed that the incidence of unplanned extubation significantly decreased from 20.45% in the control group to 2.27% in the experimental group (p = 0.007). Registered nurses demonstrated a high level of adherence to the guideline and reported satisfaction levels above 80%. The findings indicate that applying these guidelines effectively enhances patient safety and the quality of critical care nursing practice.
