Abstract

Purposes: To study the existing problems, develop, and evaluate nursing practice guidelines for the care of chronic disease patients at risk of emergency conditions at Pathumrat Hospital, Roi Et Province.

Study design: Action research.

Materials and Methods: Participants and data analysis key informants for qualitative data collection consisted of 7 nurses and healthcare personnel at Pathumrat Hospital. The quantitative sample included 17 chronic disease patients at risk of emergency conditions, 10 nurses, and 7 caregivers. Data were collected through in-depth interviews and structured questionnaires. Qualitative data were analyzed using analytical descriptive methods, while quantitative data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation. Hypothesis testing was conducted using the one-sample t-test.

Main finding: The current situation and problems in caring for chronic disease patients at risk of emergency conditions were overall rated at a moderate level (Mean±SD. = 3.13±0.70). The developed nursing practice guidelines for the care and prevention of emergency conditions in chronic disease patients consisted of four key phases: (1) risk screening and assessment, (2) proactive monitoring and care, (3) communication and emergency management, and (4) patient education and continuity of care. The overall effectiveness of care and emergency prevention was rated at a good level (Mean±SD. = 4.01±0.57), while stakeholder satisfaction including physicians, nurses, and patients was rated at a high level (Mean± SD. = 4.08±0.34)

Conclusion and recommendations: The developed practice guideline, which integrates multidisciplinary collaboration, enhances the effectiveness of care, with stakeholder satisfaction reported at good to high levels. Therefore, this guideline should be implemented as a standard in community hospitals, alongside continuous capacity building of healthcare personnel to further improve the efficiency and sustainability of the care system.
