The Effects of Stroke Care Program and the Burden of Caregivers in an Intermediate care clinic

Purposes : To examine the effects of a stroke care program in an intermediate care clinic on patients' activities of daily living, quality of life, and complications, and on family caregivers' burden and stress.
Study design : A one-group pretest–posttest quasi-experimental design.
Materials and Methods : The sample comprised 30 dyads of stroke patients and their primary family caregivers attending the intermediate care clinic at Phon Sai Hospital, Roi Et Province, selected by purposive sampling. Participants received a 3-month care program comprising health education, caregiver skills training, individual counseling, home visits, and weekly telephone/LINE follow-up. Data were collected using the Barthel ADL Index, the Stroke Specific Quality of Life Scale (SS-QOL), a complication record form, the Zarit Burden Interview (ZBI), and the Suanprung Stress test-20 (SPST-20), and analyzed using descriptive statistics, paired t-test, Wilcoxon Signed-Rank test, and McNemar test.
Main findings : After the program, patients' ADL scores increased significantly from 10.50±5.95 to 15.57±6.02 and quality of life from 127.73±35.89 to 181.73±37.57 (p<.001). The proportion of patients with at least one complication decreased from 26.7% to 3.3%. Among caregivers, caregiving burden decreased significantly from 41.50±17.69 to 16.67±12.91 and stress from 19.93±11.12 to 7.03±5.59 (p<.001).
Conclusion and recommendations: The intermediate-care stroke program significantly improved patients' ADL and quality of life, reduced complications, and reduced caregiver burden and stress. It should be applied in intermediate care clinics of community hospitals, and a controlled study with longer follow-up is recommended to confirm its effectiveness.
