The development of an Adverse Drug Reaction Monitoring System to Prevent Repeated Drug Allergies at Suwannaphum Hospital

Abstract

Purposes: To develop the Suwannaphum Unified Responsive Enhanced Adverse Drug Reaction Monitoring System (SURE-ADR Model) and evaluate its effects on adverse drug reactions (ADRs), repeated drug allergies, ADR reporting quality, pharmacists’ knowledge, and user satisfaction.

Study design: Research and development (R&D) study conducted in a community hospital setting. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation.

Materials and Methods: The study was conducted at Suwannaphum Hospital, Roi Et Province, Thailand. The SURE-ADR Model was developed based on retrospective situation analysis and literature review. The model comprised five components: Screening, Update & Alert, Review Together, Execute Safely, and Analyze & Revise. Study participants included pharmacists and patients with ADRs during fiscal year 2025. Data were collected from electronic medical records, ADR reporting systems, and questionnaires assessing pharmacists’ knowledge and satisfaction.

Main finding: A retrospective analysis of fiscal years 2019–2024 identified 453 ADR reports, of which 24.61% required hospitalization. Most reactions involved the skin and subcutaneous tissue. Following implementation of the SURE-ADR Model, 74 ADR cases were reported in fiscal year 2025. No repeated drug allergies or cross-reactive drug allergies attributable to the hospital system were identified. The ADR rate decreased from 0.47 to 0.21 per 1,000 patient-days, representing a 55.32% reduction. The number of ADR reports meeting quality criteria increased from 65 to 74 reports. Expert evaluation indicated good system appropriateness and feasibility. Pharmacists demonstrated a good level of ADR monitoring knowledge (mean score 8.08 ± 1.44) and reported the highest level of satisfaction with the system (Mean score 4.42 ± 0.52).

Conclusion and recommendation: The SURE-ADR Model improved ADR surveillance performance, reduced ADR occurrence, and strengthened medication safety in a community hospital setting. The integration of information technology, automated alerts, and multidisciplinary collaboration may enhance pharmacovigilance capacity and support the prevention of medication-related harm. Further multicenter studies with longer follow-up periods are recommended to confirm the effectiveness, generalizability, and sustainability of the model.
