Abstract

Purposes: This study aimed to identify clinical problems, develop nursing care guidelines, and evaluate the effectiveness of these guidelines in preventing Ventilator-Associated Pneumonia (VAP) in Medical Intensive Care Unit (MICU) 5 at Roi Et Hospital.

Study design: Participatory Action Research (PAR) based on the Look-Think-Act cycle.

Materials and Methods: The participants included nursing staff and patients requiring mechanical ventilation in MICU 5. The research was conducted in four phases: 1) Preparation phase (situation analysis and literature review), 2) Development phase (focus group discussions to refine the VAP bundle to fit the local context), 3) Implementation phase (pilot testing with clinical supervision and observation), and 4) Evaluation phase. The research instruments included the developed nursing care guidelines, a nursing practice observation form, and a satisfaction questionnaire.

Main findings: The results revealed that after the implementation of the developed guidelines, nursing staff demonstrated significantly higher compliance with VAP prevention protocols compared to the pre-implementation period. Consequently, the incidence rate of VAP showed a substantial decrease. Furthermore, nursing staff reported a high level of satisfaction with the guidelines, noting that they were clear, easy to follow, and addressed specific practical challenges within the unit’s context, such as modified oral care techniques for unconscious patients.

Conclusion and recommendations: Developing nursing guidelines through a participatory process fosters a sense of ownership among staff, leading to sustainable clinical practice. It is recommended that continuous clinical supervision be maintained to ensure long-term compliance. Additionally, these guidelines should be expanded to other critical care units to enhance the overall standard of safety for ventilated patients within the hospital.
