Development of service management model to reduce waiting time in outpatient department using LEAN CONCEPTS, Plapphlachai Hospital, Buriram Province

Abstract

Purposes: To examine the existing problems and contextual factors, develop and improve operational procedures, and evaluate the service delivery process to reduce waiting times in the outpatient department of Plubplachai Hospital, Buriram Province.

Study design: Participatory Action Research : PAR).

Materials and Methods: The study included 106 patients attending the outpatient department of Plubplachai Hospital, Buriram Province, and 10 multidisciplinary team members involved in the service process. Data were collected from May 1, 2025 to October 31, 2025. Research instruments consisted of a satisfaction questionnaire and an in-depth interview guideline. The questionnaire demonstrated high reliability (Cronbach’s alpha = 0.98). Data were analyzed using paired sample t-test.

Main findings: The results indicated that the average waiting time for outpatient services was 142.3 minutes, exceeding the Hospital Accreditation (HA) standard of 120 minutes. After implementing the developed service delivery model, unnecessary procedures were removed and redundant steps were integrated, reducing the process from 11 to 9 service steps. Consequently, the average waiting time decreased from 142.3 minutes to 119.01 minutes. In addition, comparisons of satisfaction levels before and after the intervention showed that both outpatients and multidisciplinary team members reported significantly higher satisfaction levels (p < .05).

Conclusion and Recommendations: The development of a service model through the application of Lean concepts can streamline processes and reduce waiting times to meet standard benchmarks. As a result, both outpatients and multidisciplinary teams demonstrate increased satisfaction. Therefore, the application of Lean principles should be extended to outpatient service units in other hospitals to minimize waste, reduce delays, and enhance value for patients. This implementation requires fostering a correct understanding of Lean concepts among personnel at all levels.
