The Development of Screening Practice Guideline for Patients with Sepsis by Systemic Inflammatory Response Syndrome Score (SIRS score), Out Patient Department, Roi Et Hospital

Abstract

Purposes: To study the situational analysis and challenges in screening patients for Sepsis, develop and evaluate the outcomes of a clinical practice guideline (CPG) for Sepsis patient screening.

Study design: Research and Development.

Material and Methods: The sample group consisted of two groups: 20 patients of both male and female genders who received services at the screening point and underwent medical treatment at Roi Et Hospital, and 10 professional nurses working at the patient screening point of the Outpatient Department Nursing Group and professional nurses working in the Internal Medicine Examination Room at Roi Et Hospital. The research will be conducted between July 1, 2025 – December 31, 2025. Experimental Instrument : The Clinical Practice Guideline for Screening Patients with Sepsis of the Outpatient Nursing Division at Roi Et Hospital. Data Collection Instruments: A questionnaire package consisting of General Information Questionnaire, Satisfaction Assessment Form and Record Form for Nursing Activities according to the Clinical Practice Guideline for Screening Patients with Sepsis. Statistics used in the analysis include descriptive statistics and inferential statistics, Paired sample t-test (p< .05)

Main finding: The problems encountered in screening Sepsis patients included: Patients lacked awareness and recognition of the risk factors. Personnel lacked sufficient skills for patient screening. The screening area and environment were unsuitable. A lack of budget and the original screening process was unclear and did not align with the time constraints for patient service delivery. The results of the development of the Clinical Practice Guideline for Screening Patients with Sepsis comprise the following steps: Step 1: Preliminary Assessment, taking the patient's history. Step 2: Risk Screening, patients with Sepsis by Systemic Inflammatory Response Syndrome Score (SIRS score). Step 3: Referral for examination and treatment. The evaluation of the guideline's implementation found that after using the guideline, both patients and service providers reported satisfaction at a high level (Mean= 2.87 and Mean = 2.97, respectively). The satisfaction of service providers before and after using the guideline was statistically significantly different (p-value < 0.05). The performance of nursing activities according to the Sepsis screening guideline was also statistically significantly different (p < .05).

Conclusion and recommendations: The development of screening guidelines for Sepsis patients by the Outpatient Nursing Department of Roi Et Hospital has resulted in clearer and more efficient screening procedures. It reduces the steps and time involved in screening, allowing high-risk patients to be referred for treatment more quickly.
