Effectiveness of Triage Training Program for Registered Nurses in Emergency Department at Pathumrat Hospital, Roi Et Province

Purposes: To examine the effectiveness of an emergency triage training program for registered nurses working in the accident and emergency department.
Study design: This study was a quasi-experimental research using a one-group pretest-posttest design. The research was conducted over a period of 8 months, from February to September 2025.
Materials and Methods: The sample consisted of 13 registered nurses and 3 Emergency Medical Technicians (EMTs) (totaling 16 personnel) working in the accident and emergency department at Pathum Rat Hospital, along with 30 patients receiving services in the department. The research procedure was divided into three phases: 1) studying the situation, personnel development concepts, and developing the emergency triage training program for Pathum Rat Hospital; 2) implementation phase; and 3) data analysis phase. Data were collected using a knowledge test, a practice skills assessment form, a satisfaction questionnaire, and a triage accuracy record form. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation, as well as inferential statistics, including the Paired t-test and Cohen's kappa, with a statistical significance level set at 0.05.
Main findings: The results revealed that after receiving the training program, the average knowledge score of the registered nurses significantly increased from 18.75 (SD. = 2.10) to 25.30 (SD. = 1.85), and the triage practice skill score significantly increased from 21.40 (SD. = 2.65) to 27.60 (SD. = 1.95) compared to before receiving the program (t = 8.42 and 7.96, respectively, p < .001). The accuracy of patient triage before the training was 70.00% and increased to 95.18% after the training. Furthermore, the agreement between the registered nurses' triage and the physicians' assessment improved from a moderate level (Cohen’s kappa = 0.56) to a good level (Cohen’s kappa = 0.82).
Conclusion and recommendations: The developed training program effectively enhanced patient triage knowledge and practice skills, resulting in increased triage accuracy. Consequently, the mis triage rate and the incidence of patients' clinical deterioration while waiting for examination decreased. This program should be promoted for implementation in healthcare facilities with similar contexts, and long-term outcomes should be further investigated.
