Effectiveness of Clinical Nursing Practice Guideline for Patients with Knee Osteoarthritis who Undergoing Total Knee Replacement (TKR) at Roi Et Hospital

Abstract

Purposes: To compare pain levels, range of motion (ROM), and complications between patients receiving a newly developed clinical nursing practice guideline (CNPG) and those receiving conventional nursing care, and to evaluate nurse satisfaction with the guideline.

Study design:  Quasi-experimental research.

Materials and Methods: The sample consisted of 32 patients undergoing total knee replacement (TKR) at Roi Et Hospital, divided into an experimental group and a control group (16 patients each). The intervention was an evidence-based CNPG covering pre-operative, post-operative, and pre-discharge phases. Data collection tools included the Numerical Rating Scale (NRS) and a goniometer. Data were analyzed using descriptive statistics and independent t-tests.

Main finding: After implementing the CNPG, the experimental group had significantly lower pain scores and higher knee flexion degrees compared to the control group (p < .05). At discharge, the experimental group achieved a maximum mean flexion of 120 degrees (SD. = 0.00). No complications occurred in either group. Furthermore, nurses reported a very high level of satisfaction (M = 47.65, SD. = 2.49), with the highest score in the confidence that the CNPG improves clinical outcomes (M = 5.00).

Conclusion and Recommendations: The developed CNPG is effective in pain management and enhancing knee range of motion, leading to rapid and safe recovery for TKR patients.
