Development of Nursing guidelines for Sepsis Patients in Emergency Accident and Forensic Department Selaphum Hospital, Roi Et Province

Abstract

Purposes: To develop nursing practice guidelines and evaluate clinical outcomes of their implementation at Selaphum Hospital, Roi Et Province.

Study design: An operational research study.

Materials and Methods: The study sample consisted of two groups: (1) 32 of healthcare personnel involved in the nursing care of patients with bloodstream infections, and (2) 45 patients diagnosed with bloodstream infections. Data were collected using a knowledge test, a nursing practice questionnaire, and a clinical outcome record form. The questionnaire had a content validity index (CVI) of 0.87. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation, as well as inferential statistics, namely the paired t-test and Chi-square test.

Main findings: The developed nursing practice guidelines comprised six key components: 1) screening and early diagnosis, 2) assessment and monitoring, 3) initial treatment within 1 hour, 4) ongoing management, 5) complication prevention, and 6) referral system management. Implementation significantly reduced mortality rates (28.9% to 15.6%, p < .05), shortened hospital stays (9.8 to 7.2 days), and increased compliance with timely antibiotic administration (65.5% to 89.4% within 1 hour post-diagnosis).

Conclusion and Recommendations: This study successfully established evidence-based nursing guidelines that significantly improved sepsis care outcomes. To ensure sustainability, we recommend: 1) hospital-wide and regional dissemination of the protocol, 2) continuous staff competency training programs, and 3) longitudinal outcome evaluations. These measures will optimize sepsis management across healthcare networks while maintaining treatment efficacy.
