The Development of Local Health Insurance Fund Model in Phichit Province

Abstract

Purposes : To examine the factors related to fund management and to develop a management model for the Local Health Security Fund in Phichit Province.

Study design : Mixed Methods Research.

Materials and Methods : The sample group consisted of personnel responsible for tasks and managers of the Local Health Security Fund project in Phichit Province. The quantitative research involved 202 task officers, while the qualitative research involved 16 local administrative organization managers. The research tools were questionnaires that gathered general information, knowledge, attitudes, and operational data, as well as interviews to collect opinions on the problems, obstacles, and suggestions for the implementation of the Local Health Security Fund project in Phichit Province. Data were analyzed using frequency, percentage, mean, and standard deviation, with Chi-square statistics and binary logistic regression. The qualitative data were summarized using content analysis.

Main findings : The factors associated with the implementation of the National Health Security Fund framework by local administrative organizations are the agencies (AOR=2.32, p < .05), those who do not use the operational manual (AOR=2.57, p < .05), those who do not use the group chat (AOR=2.22, p < .05), and knowledge (AOR=6.65, p < .001). The design of activities in the local health security fund development project revealed that the main gaps are in the understanding and knowledge details. Therefore, learning activities should focus on active learning, skill development, and prioritization (priority setting) based on specific or general content, and specific or general skills. This approach will stimulate engagement in learning and determine the intensity of activities, allowing learners to remember, understand, analyze, and apply the knowledge. Responsible personnel will develop positive attitudes (Affective Domain) and acquire the necessary skills (Psychomotor Domain), which will enable the local health security fund project to effectively cover target groups and meet the criteria of the National Health Security Fund program.

Conclusion and recommendations : Relevant agencies should organize active learning activities for those responsible for the tasks, in order to stimulate learning and enhance the necessary skills for managing the Local Health Security Fund.
