Effects of a Case-Based Management Nursing Care Plan on Symptom Pain and Perceived Quality in Palliative Care of Patients

Purposes : The study aims to examine how a Case-Based Management Nursing Care Plan influences symptom pain and to investigate its impact on the perceived quality of palliative care in patients.
Study design : The study design involves a single-group experimental approach comprising both pre-test and post-test assessments.
Materials and Methods : This study was conducted between July and November 2023 at Thawatchburi Hospital in Roi Et province. A purposive sampling method was employed to recruit 30 adult patients who met specific criteria: 1) diagnosed with terminal illnesses and referred by their physicians to the palliative care center: 2) informed by their physicians about their terminal illnesses and advised to undergo palliative care; and 3) possessing full mental capacity. The program involved five stages: 1) identification and selection of patients; 2) assessment and examination of their issues; 3) formulation of case-based care methods tailored to identified problems; 4) execution of the program as per the devised plan; and 5) evaluation of program outcomes. Data collection involved three methods: 1) extraction from the patient's general records; 2) utilization of the Thai version of the Edmonton Symptom Assessment System; and 3) implementation of the Thai version of the Palliative Care Outcome Assessment Scale. Validation tests using Cronbach’s alpha coefficients were conducted for both the Edmonton Symptom Assessment System and the Palliative Care Outcome Assessment Scale, resulting in scores of 0.76 and 0.73 (n=20), respectively. Descriptive statistics were applied for the analysis of general data, while paired t-tests were employed to compare perceptions of symptom-induced suffering and palliative care outcomes.
Main findings : Following program participation, patients exhibited a notably reduced average score in symptom-induced suffering (t=11.793, p<.001) and a significantly elevated average score in the perception of palliative care quality (t=-44.819, p<.001).
Conclusion and recommendations : The utilization of this case-based nursing care plan is recommended in palliative care centers as an effective approach for mitigating symptom pain and enhancing the perceived quality of palliative care for patients.
