Promoting Health Management of the Elderly with Chronic Diseases: A case study of dependent elderly people

Abstract

Purposes : To promote health management in elderly people with chronic diseases and dependency conditions.

Materials and methods : A study to compare two dependent patients with chronic diseases receiving services at the community health center, Phon Hospital, Phon District, Khon Kaen Province. Conducting a study Between 7 June 2023 and 2 October 2023, the tools used include: Data collection form Patient medical records, interviews, observations, data analysis using the Gordon Health Model. Disease pathology, treatment, nursing planning and operations according to nursing diagnosis. and evaluate nursing results.

Main findings : It was found that both patients had the same problem, which was complications from chronic disease. Decreased ability to perform daily activities Risk of falls and caregivers lack awareness of taking care of patients appropriately for their disease Patient 1 had good awareness of his own health condition. But there was a lack of motivation and motivation to take care of and restore health. Be prepared for caregivers This makes the patient's health condition likely to improve. Patient 2 had communication and cognitive problems due to dementia. In nursing, educators must provide guidance and empowerment. Mainly the caretaker but the caregiver was an elderly person and had a chronic disease. Tired causing impaired care.

Conclusion and recommendations : The results of this research indicate that the participation of families, communities and related agencies will help patients have more opportunities to access continuous care.
