The Development of Discharge Planning Model for Stroke Patients with Intermediate Care in Inpatient Ward at Thawatchaburi Hospital

Purposes : To explicate problem situations, develop models, and explicate results of discharge planning model for stroke patients with intermediate care inpatient ward, in Thawatchaburi Hospital.
Study design : Action research Approach.
Materials and Methods : Twenty-one of key informants were a stroke patient receiving treatment at the inpatient facility in Thawatchaburi Hospital, 21 relatives and caregivers, and 18 multidisciplinary teams. From July to November 2023, data were collected using questionnaires, and analyzed the data using frequency, percentage, mean, standard deviation, and percentage difference.
Main findings : 1) Most of physical impairment problems were physical movement problems, with 20 cases (95.0%) having complications, A.D. 2020-2022, 3 cases, 1 case, and 1 case, respectively. 2) The intermediate care discharge planning model for stroke patients consists of 7 steps: (1) registration, (2) Admission to Saharak rehabilitation room and assessment of the patient/family, (3) analysis of problems and needs. patients and relatives (4) set goals for rehabilitation care, (5) plan nursing care, (6) assess readiness, and (7) discharge patients, and 3) after intervention it were found that Mid-stage stroke patients had differences in their ability to perform daily activities (ADL) increased from 14.23 to 17.76, an increase of 22.20%. Re-Admit rate with stroke. Complications within 28 days decreased by 200.00%.
Conclusion and recommendations : The results of this research indicate that intermediate care inpatient ward planning model for stroke patients results in patients having increased ability to carry out their daily lives. Therefore, relevant agencies should be encouraged to adopt model. This is to be carried out in the area.
