Development of Care Systems for Sepsis in Emergency Room Thawatburi Hospital

Abstract

Purposes : To study o study the conditions and problems, develop systems, and evaluate results for sepsis in emergency room Thawatburi Hospital.

Study design : Action Research (AR).

Materials and Methods :  All total of 40 key informants were doctors, nurses and  related persons and medical records 144 of patients diagnosed with sepsis who came to emergency services from October 2022 to September 2023. Data analyzed by using frequency, percentage, mean and standard deviation.

Main findings :  1) Study of problem found that problems of personnel, processes, and patients services. 2) Solutions include: (1) develop nursing clinical practice guidelines (NCPG) (2) develop symptom assessment skills using a q-SOFA assessment (3) triage (4) SBAR doctor reporting (5) procurement of necessary medical materials and equipment, and (6) monitoring, supervision, and evaluation of compliance with specified guidelines, and 3) The development results found that number of patients with bloodstream infections who came to receive services increased, accurate patient assessment and diagnosis have increased. Nursing quality results, they were found that misclassification of patients (Under triage) decreased, symptoms deteriorated, referrals within 2 hours after admission decreased, and no patients were found to have died.

Conclusion and recommendations : The results of this research result in increased quality of the care system for patients with bloodstream infections. Therefore, this system should be used in the agency.
