The Development of Continuity of Care Model with Community Participation for Bed Bound Elderly at Muang Roi-Et

Abstract

Purpose : To development of continuity of care model with community participation for bed bound elderly at Muang Roi-Et.

Study design : Action research design.

Materials and Methods : We obtained 35 from the key informants;13 were professional nurses, were 7 bed bound elderlies who needed assistance in daily activities, were 7 relatives or careers, and were 7 community network partners who were community health volunteers, one person per community.The period of operation was from October, 2022 to August, 2023. Data were collected using the interview questionnaire, questions for group discussions participation observation form activity record form. Data were analyzed using frequency, percentage, mean, standard deviation and paired t-test.

Main findings : A model of continuous care with community participation for bed bound elderly, Six were processes 5P1E model were divided in 1) P : Prepare 2) P : Policy  3) P: Project 4) P: Process 5) P: Participation 1E: Evaluation. And after intervention, it was found that (1) caregivers had a higher mean score of knowledge and understanding about caring for bed bound elderlies than before training (p<.004); (2) The mean score of caring skills for bed bound elderlies were more than 80% and (3) Two bed bound elderlies had good quality of life, representing 28.60 % and 5 moderate cases, representing 71.40%.However, it was noteworthy that none of bed bound elderlies had poor quality of life. In terms of community participation, knowledge, attitude and overall practice in elderly health care were at a high level.

Conclusion and recommendations : The results of this research, it resulted in subject group having knowledge, understanding, caring skills, and bed bound elderlies had a better quality of life.
