The Development of Nursing Practice Guidelines for Percutaneous Nephrolithotomy of Patients in Roi-Et Hospital

Abstract

Purposes : To develop and study results a nursing practice guideline for percutaneous nephrolithotomy (PCNL) of patients in Roi-Et Hospital

Study design : Research and Development study.                                   

Materials and Methods : All total of 20 subjects were professional nurses working at operating room and 20 of percutaneous kidney stone surgery patients at Urology Department. This research were divided into 4 phases: 1) finding clinical problems, 2) searching for evidence, 3) trialing the guideline, and 4) implementing the updated guideline in the organization. Data were collected using a questionnaire and descriptive statistics were used to analyze for data.

Main findings : The study subjects consisted of 20 operating room nurses, 65.0% of which were female, 60.0% had worked at the operating room for less than 10 years. The guideline from the development consisted of 10 steps: 1) to reduce anxiety about surgery, 2) to prevent surgery on the wrong side, 3) to prevent accidents from moving, 4) to preventing injuries from a positions, 5) to prevent allergies to drugs or chemicals, 6) to prevent the danger from using surgical instruments, 7) to prevent the harm to kidneys and nearby organs that affect the blood loss of more than 500 cc, 8) to prevent the occurrence of hypothermia, 9) to prevent the occurrence of Infection in the bloodstream, and 10) promote correct behavior after surgery. The results of the practice were not found for complications in the preoperative, operative, and postoperative stage. It can actually be implemented and produces positive results for patients at a high level. There was an overall satisfaction of guideline users at a high level.

Conclusion and recommendations : The development of this nursing practice guideline resulted in positive outcomes with patients and users of the practice at a high level of overall satisfaction.
