The second patient underwent surgery to remove the blood clot within 6 hours after the accident. Have a pneumonia due to mechanical ventilation for a long time, because the patients are unconscious, 2 patients have 4 nursing diagnoses were consistent, but 8 were different 2) Nursing practice consistent  with problems in 3 phases of care according to problems from assessment and diagnosis in nursing.
