Comparative Effect of a Medication Reconciliation on Medication Error in IPD Paperless KPHIS for Chronic Disease in Lahansai Hospital

Abstract

Purposes: To compare the coverage of the Medication Reconciliation process. And medication errors. Before and after using the IPD Paperless KPHIS system in patients with chronic diseases while hospitalized.

Study design: This study is a retrospective research study.

Materials and Methods: The sample group consisted of all chronic disease patients admitted to Lahansai Hospital from 1 April 2023 to 31 March 2024, a total of 1,550. Data were collected from the HosXp program. That using the Medication Reconciliation data collection form. Data were analyzed using the Independent Chi-square test, process both in admit and discharge patients from the hospital.

Main findings: Medication Reconciliation in 1,550 patients. divided into 754 patients before the operation in the IPD Paperless KPHIS system and 796 patients after the operation. Coverage of the operation Medication Reconciliation Increased from 97.9 percent to 99.5 percent with statistical significance (p = .005). Medication errors decreased from 5.6 percent to 4.4 percent (p = .228). Medication errors resolved by pharmacists increased from 93.2 percent to 94.3 percent (p = .231). The most common of medication error did not order for continues medication before and after the system change, 52.3% and 54.3%, respectively.

Conclusion and recommendation: Development of system to Implement of the IPD Paperless KPHIS system that found Medication Reconciliation offers significant increased coverage both in the process of admit and discharge patients That allows the patient to receive continuous medication and reduce the occurrence of medication errors and adverse events. And allows patients to continue receiving medication across treatment boundaries.
