Nursing Care for Patients with End Stage Renal Disease form Continuous Ambulatory Peritoneal Dialysis : Case study 2 Case

Abstract

Purposes : To study nursing care for patients with end-stage chronic kidney disease who receive kidney replacement therapy using continuous peritoneal dialysis: 2 case studies.

Material and Methods : This was a comparative study of patients with end-stage renal failure. Two patients who received continuous peritoneal dialysis were admitted for treatment in the outpatient department of Phon Hospital, Phon District, Khon Kaen Province.  Tools used in the case study consist of a data recording form which data were collected from outpatient medical records, interviewing relatives, formulate nursing diagnoses using patient assessment concepts based on health patterns of Gordon's 11 patterns plan and perform nursing actions according to nursing diagnosis and evaluate nursing results.

Main finding : It was found that two patients with end-stage chronic kidney disease who received continuous peritoneal dialysis had the same nursing problem, namely, waste buildup in the body due to the kidney unit's function deteriorating. There is anemia due to decreased production of erythropoietin, have low levels of protein in the blood, can't sleep well and anxious about illness. The difference in nursing care is that the first patient has chronic obstructive pulmonary disease. Both lungs have a whistling sound. The second patient had co-morbidities such as diabetes and high blood pressure. Therefore, it has high blood pressure, hyperglycemia and hypokalemia.

Conclusion and recommendations : This study shows that nurses must have the knowledge and skills for comprehensively assessing symptoms and main symptoms and observing unusual symptoms.
