Development of Initial Assessing the Traumatic Head Injury Patients and Nursing at Emergency and Forensic Unit Phen Hospital, Udonthani

Abstract

Purposes : To study the situation, develop systems and evaluate results on Initial assessing the traumatic head injury patients and nursing.

Study design : Mutual collaborative action research.

Materials and Methods : Ten of key informants were patients with head injuries in the situation study phase and 20 of the service system development phase who met the inclusion and exclusion criteria. The research was conducted from January to September 2023. Data were collected using patient record forms, performance evaluation form, assessment of patient condition and questions for focus group. Data were analyzed using frequency, percentage, mean, and standard deviation and content analysis. 

Main findings : (1) Found that the problems with service personnel and service systems/communication equipment (2) developed a system for assessing the condition of head injury patients first admitted for treatment. The development process were carried out in 2 loops, resulting in nursing guidelines consisting of 9 categories, with a possibility of practice accounting for 93.3%, and (3) development results found that the patient had a Glasgow score on admission of 13-15 scores (95.0%) and upon discharge 13-15 scores (95.0%), cranial radiographs (80.0%) were normal (75.0%). The diagnosis were mild traumatic, brain injury (95.0%), but there were severe traumatic brain injury (5.0%) and only 60.0% of patients were hospitalized.

Conclusion and recommendations : This research allows patients with head injuries to receive an accurate evaluation of their condition upon admission, thus providing a guideline for nurses to use in nursing practice to ensure safety and timely care.
